The Conrad Pearson Clinic recently upgraded to an electronic check-in system. There is no longer a need to fill out paper forms. You will be receiving an e-mail or text message with a link to complete your registration prior to your appointment. This will save you time and eliminate the need for you to complete any paperwork when you arrive. You can use any smartphone, tablet or computer to register from the privacy of your own home.

You can also check in on your own device when you arrive. Just make sure we have your up-to-date email address and cell phone number when you make your appointment and we’ll make your next visit simpler and faster.
